Bedtime Story 


A Chimpanzee's Apology


I am extremely sorry more for myself than for you my cousin apes that I was unable to publish the bedtime story for the last night.

Of course there is always the enticing route of cheating, that is, to publish as a matter of record on a back date on my blog page but that would drastically bring down the nobility rating of this insignificant average ape.

It is thanks to my brilliant brother ape (yes, among the cousins there is a degree of genetic separations) that not only am I able to write down a bedtime story tonight but also managed to resuscitate from the dead all the data of my patients and update them with those who followed suit after the Windows 10 crash.

I must once again clarify what I mean by cousin even though I may have mentioned it earlier in one of my bedtime stories.

Any two people who share a common ancestor are cousins and so my brother is my closest cousin in a manner of speaking.

But my brother does not satisfy the other two prerequisites that are required of being a cousin.

Firstly to qualify to be cousins two people must not share even a single parent and secondly they should not be a sibling of a common ancestor.

The second condition rules out relatives such as uncles or aunts and nephews or nieces from being labeled as cousins.  

Thus a brother cannot technically be called a cousin.   

So that was a little bit of explanation of the eventful evening of yester night that got me pretty rattled and shaky and I learnt that one needs to save data more diligently, preferably everyday and ideally on Google Drive rather than some kind of physical device. 

So let resume the story of couching – the highly prized surgical technique of ayurveda that till today finds its mention in some of the textbooks of Ophthalmology.
  
For performing couching the ideal site on the eye ball is the Pars plana which is the Latin for “flat portion”.

Para plana is anatomically a part of the ciliary body (that produces aqueous humor) which in turn is part of the middle vascular layer of the eye ball called uvea.

Par plana is an area that lies just beyond the transparent cornea at about 3 to 3.5 mm into the sclera.

It is obvious that these ancient surgeons were unaware of the anatomy of the eye and this “safe site” must have been arrived at after a series of trial and error.      

It is an ideal site for surgeons to enter the posterior vitreous cavity since it serves no function after the post-fetal period, is avascular and does not bear upon it the ten distinct layers of retina.

In couching this route was used to shove the lens behind.

In this process not only is the lens dislocated from its secure place but also its secluded place exposing its antigens to the body’s immune system since the poking itself disturbs the natural barriers that barricades away blood and its plethora of immune cells and chemical mediators.
